GENERAL INFORMATION for GUIDE of PATIENTS & CUSTOMERS

ADMISSION PROCEDURES

Present Doctor’s Order for admission and PHILHEALTH Requirements.

Present Senior Citizen ID and Purchase Booklet or PWD card.

Choose your room accommodation.

Read thoroughly, completely and accurately fill-up the Patient Data Sheet, the Consent for Care for Confinement and the Contract for Confinement (Terms incident to Patient Admission) before affixing your SIGNATURE.

After completion of admission documentation, the Admitting Section Personnel will issue a Patient ID band which must be worn within the Hospital premises during the patient’s hospital confinement.

INITIAL PAYMENT

Patient will be asked for an initial payment depending on the room of choice and case type for NON-EMERGENCY cases only. This amount will be deducted from your final bill.

Statement of Accounts or Outstanding Hospital Bills can be requested from the Billing Unit or Credit and Collection Unit for patients to prepare and pay their Hospital and Doctors’ bills promptly.


HEALTH MAINTENANCE ORGANIZATION and CORPORATE ACCOUNTS

Patients of accredited companies with the Hospital shall submit a Letter of Authorization (LOA) or GUARANTEE LETTER (GL) from the company, otherwise, the Hospital Bills will be attributed to the personal account of the patient.

All billing inquiries must be directed to Billing Unit at local 1108 and Credit and Collection Unit at local 1111, Ground Floor.


PHILHEALTH

Philhealth Members must inform the Admitting Section of their membership.

Philhealth Claim Form 1 can be obtained from the Admitting Section.

Philhealth CARES Nurse is available in the Admitting Section for any inquiry and clarification.

Philhealth Benefits Unit can be contacted at local tel no. 1114 or you may see visit their office at the Main Accounting - Ground Floor.


NURSING SERVICE

All Private Rooms are equipped with a Nurse’s Call Button, Press this when you need assistance from the Nurses.


VALUABLES

Do not leave jewelries, money, gadgets and other valuables UN-ATTENDED. Safekeeping of these valuables is only allowed for patients on emergency case and with no companion. This will be facilitated by the Hospital Cashier.


TELEPHONE SERVICE

LOCAL OUTSIDE Call can be done by dialing “9”. Long distance for Domestic and International calls are not allowed.


DIETARY SERVICE

Professional dietitians are in charge of preparing patient meals. Depending on the recommendation of the patient's physician and in consultation with the dietitian, a diet program specifically catering to the needs of the patient may be made. Meals are served on the following schedule:

Visitors and guests are welcomed to bring cheer and comfort to patients, however, they are required to strictly follow the Hospital Visiting Hours to ensure the well-being and safety of both the patient and visitors:

Children below seven years are not allowed inside the Hospital unless scheduled for medical check-up.

Overcrowding and Overstaying are not allowed.

Visitors must comply with the Health and Safety Programs of the Hospital.


NO SMOKING POLICY

Smoking is strictly prohibited throughout the Hospital premises.


FIREARMS and DEADLY WEAPONS

Carrying of firearms and deadly weapons inside the Hospital is STRICTLY PROHIBITED. For those authorized or with issued firearms, you are instructed to DEPOSIT your firearms to the Entrance/Lobby Security Guard for safekeeping.


WASTE SEGREGATION AND CONSERVATION OF RESOURCES

The Hospital practices sound environment policy by observing the Waste Management and Conservation of Energy and Water. Washing of laundry and Cooking are not allowed.


DISCHARGE PROCESS

Your Attending Physicians must write the Order for Discharge or May go Home in the Patient’s Chart.

The Nursing Staff will inform the Billing or Credit and Collection Unit of the discharge order from the doctors.

The Billing Staff will process and finalize your Hospital Bills.

Customers will be called when the bill is ready for payment.

Please review your Statement of Accounts before paying.

Outstanding Balance of patient’s accounts may be inquired from the Billing or Credit and Collection Unit from 7:00AM to 7:00PM.

The cut-off time for discharging of patients is at 12:00NN and 6:00PM.


SETTLEMENT OF ACCOUNTS

Please settle your account at the Cashier located at the Ground Floor and is opened daily for twenty four (24) hours.

Always ask for the Official Receipt for all payments made.

After settlement of accounts, customers will be given Release Forms which will be presented to the Nursing Staff and Lobby Guard.

For those needing a place or extra room for your family and relatives, may we refer you to the Fray Juan Clemente Holistic Center located at the SJDEFI College , adjacent to the Hospital. For inquiries, please call 551-4384 / 551-4386.


PATIENT SATISFACTION INDEX (PSI)

Please let us know your comments and suggestions through the PSI. Your feedback will be treated with utmost confidentiality and will be used for the evaluation and improvement of our staff and our services.

PSI forms are available at the Nurse's Station. Please accomplish the survey questionnaires and leave it with the nurse on duty. For more information, please call the Quality Assurance Office at local 1224.
